I would like to enroll a dependent in our HMO. How do I do it and how much would it cost me?

You can only add a qualified dependent under our HMO following the hierarchy.

Here is the list of qualified dependents and the hierarchy:

For Married Employees:
1. Spouse (up to 65 years old)
2. Child/ Children (15-days old up to 25 years old)

For Single Employees with Child/ Children:
1. Child/ Children (15-days old up to 25 years old)
2. Parents (up to 65 years old)
3. Siblings (15-days old up to 25 years old)

For Single Employees without Child/ Children:
1. Parents (up to 65 years old)
2. Siblings (15-days old up to 25 years old)

For employees based in Visayas and Mindanao
For employees based in Luzon
