How do I apply for an SSS Sickness benefit claim?

You must notify People Ops of your sickness or injury by filling out the SSS Sickness-Benefit Notification form within within 5 calendar days from the start date of confinement, either for home or hospital confinement.

Here are the requirements needed aside from the accomplished SSS sickness form:

For home confinement (original copy):
- Medical Certificate

For hospital confinement (original copy):
1. Laboratory
2. Xray
3. ECG (if applicable)
4. Discharge Summary and other diagnostic results
5. SSS cardmember/ Valid IDs
6. Medical Certificate (With doctor's license number and signature)
